LiveOnNY Foundation: Our Mission
The LiveOnNY Foundation supports innovative initiatives, benefitting both donor families and transplant patients through their journies. Making a contribution to the LiveOnNY Foundation helps us to provide the greatest gift of all – the gift of life.
About Our Programming
Since the LiveOnNY Foundation was created in 2016, the Foundation has awarded over $1.5 million in grants supporting programs such as: Advanced Donor Management Fellowship, Donate Eight Simulation Center, Painted Legacies, Sean's Gift Blanket Program, Gift of Life Transport, The Marino Prodan Legacy Award and others. We continue to review and support grants from the donation and transplantation community. For additional information or questions, contact: Jenny Lai, Executive Director of the LiveOnNY Foundation at info@liveonnyfoundation.org.
Donate Eight Simulation Center at LiveOnNY

Unveiled in 2024, this cutting-edge Simulation Center elevates training and education. The Donate Eight Simulation Center, located at the LiveOnNY Headquarters in Queens, is state-of-the-art and advances excellence in organ donor care through trainings for LiveOnNY clinical and field staff and education for the public on the lifesaving process of organ donation. The Center includes a fully equipped mock Operating Room and mock Intensive Care Unit designed to train employees in donor case management within a controlled and immersive environment. To date, LiveOnNY has welcomed hundreds of visitors, including high school and college students, state representatives, and many others. The Simulation Center has become an incredible resource – not only for training our staff, but also as a highly effective tool for community education.
Advanced Donor Management Fellowship

Established in March 2025, NYU Grossman School of Medicine and LiveOnNY launched the first-ever Advanced Organ Donor Management Fellowship in the United States, one designed to transform the field of medical education in organ donation. The fellowship is an innovative new medical discipline, focused on organ donation to provide donor hospitals and health systems with a dedicated specialist trained to optimize a donor's medical status, ensuring that organs are procured under the best possible conditions. The initial funding for this first-ever fellowship is provided by the LiveOnNY Foundation and will support a medical trainee for three years as part of NYU Langone’s resident fellowship program. The fellow will participate in education, clinical rotations, and research within NYU Langone and LiveOnNY in the New York region, to create a teaching platform for best practices in organ donation collaboration across different care teams and various hospital settings.
Supporting Donor Families Through Foundation Programming

Painted Legacy
The Painted Legacy events are therapeutic and commemorative gatherings which honor the legacy of organ and tissue donor heroes. Bringing together donor families, transplant recipients, and community members, the event provides a meaningful space for reflection, healing, and connection. Attendees honor their loved one by creating a paint-by-number canvas of their loved one and allow to attendees to share their stories.
Sean's Gift Blanket Program
The Sean's Gift Blanket Program was founded in memory of Sean, a courageous donor hero who saved three lives through the gift of organ donation. Families are presented with hand-knitted blankets—symbolizing warmth, comfort, and the enduring impact of Sean’s selfless act.
Gift of Life Transport
Gift of Life Family Transport focuses on providing transportation services to organ donor families who are going through the sudden loss of a loved one. The Gift of Life Family Transport was started when the Forsyth family lost their daughter Amanda in a car accident. They were provided 24 hour transportation services, a gesture that made all the difference for a family suffering the devastating loss of a child. This service allowed for Amanda’s family to be with her as much a possible during her hospitalization, relieving them the stressful task of driving during their grief. Pictured here is William Dacunto, Chair of the LiveOnNY Foundation, presenting a check to founder and donor mom, Diane Forsyth.
Marino Prodan Legacy Award
Now in its second year, the Marino Prodan Legacy Award supports ten children each year who have lost a parent, sibling or other close relative who gave the gift of life to others.
Supporting Transplant Recipients on Their Journey

Through it’s “Ya Gotta Have Heart” grant program, LiveOnNY Foundation has supported more than 700 transplant families and patients like little Izabella since 2017 (pictured with her mother). Harboring Hearts provides emergency housing, transportation and food to low income patients and their families across our eight partner transplant centers. Izabella is a heart transplant recipient and her family had one less worry going through the process by having access to a grant from Harboring Hearts. Today, Izabella is thriving and she and her family help share the message about the importance of signing up as an organ donor.
