Individuals, teams and organizations will be spending one day focused solely on getting New Yorkers to enroll as organ donors.

why is this important?

New York is 50th out of 50 states in the percentage of residents enrolled as organ donors. This isn’t the New York we know. The New York we know isn’t last in anything, especially when it's something that includes helping others. Every completed enrollment form could help save the lives of up to 8 people.

how does it work?

Participants will have one goal: enroll as many willing New Yorkers as possible.
  • New Yorkers will collect enrollments as an individual or a team.
  • Organizations will show support in a variety of ways, such as: creating their own teams, hosting a drive and/or promoting the cause within the organization.

You can enroll as many New Yorkers as you want within the following areas:

  • Manhattan
  • Queens
  • Brooklyn
  • Bronx
  • Staten Island
  • Westchester
  • Rockland County
  • Dutchess County
  • Orange County
  • Putnam County
  • Long Island

How to Participate - Individuals/Teams

On Wednesday, Oct. 4th, 2017, we are asking you to help us with one goal: enroll as many willing New Yorkers as possible!

 

GETTING STARTED IS EASY! HERE'S WHAT YOU NEED TO DO:

Step 1: Register as an individual or team (Registration opens this summer!)

Step 2: On October 4, collect enrollments by tabling in public, hitting the streets, taking forms to work, calling friends or other creative (and appropriate) ways.

Step 3: Collect as many enrollments as possible. 

Step 4: Enjoy yourself and feel good knowing your work is saving lives in the future.

 

 

CREATE A TEAM

When forming a team, each team captain will be responsible for creating a team name. The team name should be in honor of someone touched by donation or any description that best fits the people on your team. Your team name will follow the format of Long Live __________. Some examples of team names could be Long Live Josh or Long Live Brooklyn. 

 

PROVIDED MATERIALS

Materials will be provided to participants in advance of the event. Materials can be picked up in person or sent by mail.

Participants will be provided with the following:

  • Information about the day
  • T-shirts*
  • Enrollment forms and materials
  • Promotional Tool Kit, to include items such as social media posts and flyer templates

 *T-shirts available while supplies last.

 

SUBMISSION INFORMATION

All completed forms will need to be submitted on Oct. 5. You can submit forms at one of our dropoff locations or via mail. A final list of dropoff locations will be available online no later than two weeks before the event and will be emailed to all participants. If you plan on mailing in enrollments, arrangements will need to be made ahead of time, so LiveOnNY can provide you with preaddressed envelopes or labels. All mail submissions will need to be postmarked for Oct. 5. Information will be provided in your instructional packet.

RULES AND DISCLAIMERS

By participating in the 2017 Organ Donor Enrollment Day, the following rules and regulations will be in effect.

How To Enter/Eligibility

  • Participants must register through the appointed registration website as directed by LiveOnNY on the event website, www.enrollmentday.org.
  • For a participating team, the Team Captain will be the main point of contact and will represent the entry for the full team.
  • Teams and individuals will be competing against each other to drive the most enrollments. The team doing so will recieve public recognition and a plaque. Teams and individuals not associated with an organization will not be competing against any organizations. They will be competing in their own categories for bragging rights. 

 Eligibility of Enrollment Forms

  • All participants must obtain enrollments in appropriate ways and legal ways. All people filling out enrollment forms must be doing so on their own free will. Registrants cannot be paid for their enrollment.
  • All required information on the forms must be complete and legible to be eligible. Incomplete forms will not be counted.
  • Enrollment forms to be used can only be those provided by LiveOnNY through direct mail, in person pick-up or online downloadable PDF as provided on www.enrollmentday.org website. To be counted as eligible enrollments, all forms must be completed and dated for 10/04/17.
  • For enrollments to be counted towards the appropriate team, a pre-assigned tracking code (provided by LiveOnNY) must be written on the form.
  • All enrollments must be obtained within the following areas in New York State: Rockland County, Westchester County, Dutchess County, Orange County, Putnam County, Long Island, NYC (5 Boroughs)
  • LiveOnNY is not responsible for late, lost, illegible, misdirected or incomplete enrollments.

 Prizes

  • Recognition plaques will be awarded to the team that collects the most physical enrollment forms.
    • The numbers will be calulated based on the number of enrollments per team member. For example, a team of 5 people that submit 100 completed enrollment forms will be counted on their average of 25 (100/5 = 25). This would compete directly iwth the total number of completed enrollment forms by a single individual. 
    • Recognition plaques will be awarded to the winners at the LiveOnNY Volunteer Reception

 Winner Selection and Notification

  • All enrollment forms will be vetted to ensure validity. Winners will be announced upon completion of the review by the LiveOnNY and notified via contact information provided. 

 General Conditions

  • By entering, participants agree to be bound by these competition rules and the final decisions of LiveOnNY. LiveOnNY reserves the right to modify the terms of the competition at any time without prior notice. LiveOnNY, reserves final judgment as to all matters concerning competition and interpretation of the competition rules.
  • Participation and registration in the event constitutes permission to use winner’s information and or winning results on LiveOnNY’s website and its affiliated social media platforms.
  •  Anyone using fraudulent means to participate and garner enrollments in order to win the competition will be disqualified.

 

 

 

How To Participate - Organizations

On Wednesday, October 4th, 2017, we are asking you to help us with one goal: enroll as many willing New Yorkers as possible to save lives!

A LiveOnNY staff member will work with you to help your organization participate in Organ Donor Enrollment Day. Here are some ways you can participate prior to Oct. 4:

  • Promote the event through your organization's newsletter, email, website and/or socia media to generate excitement
  • Host a donor family member or recipient to speak in office

Here are some ways you can participate on Oct. 4:

  • Creating a team to go out and collect enrollments on behalf of your organization
  • Running an enrollment drive at your location
  • Encourage staff members to sign up themselves, then serve as an advocate to sign others up as well
  • Serving as a drop-off location for individuals'/teams' forms
  • Making an in-kind donation to support the day’s efforts

How much does it cost to participate in Organ Donor Enrollment Day?

It’s free! There is no registration fee for participating in Organ Donor Enrollment Day. You will be provided with all the materials you need to go out on October 4th and get people signed up.

 

Any additional promotion initiated by an individual or organization is possible, but would be at the expense of the individual or organization.

How can I register myself or my team?

When registration opens this summer, a link to the events page will be available on this site. Check back!

Is there an age restriction for participants?

There are no age restrictions for participants. 

How do I get the provided materials?

Information will be emailed to you before Oct. 4 on how and where you can pick up your materials.

What materials are provided to me and/or my team?

You will receive a t-shirt (or t-shirt per team member), enrollment forms, pens, and all the information you will need for the day.

Are there limitations on team size?

There are no limitations to how many members are on a team. Each member must be registered on the Organ Donor Enrollment Day Registration Page, which will launch this summer.

I am a team captain. Can I register members of my team for them?

Yes. Please make sure you have each individual’s details. After you create a team, you can begin registering other members. You will also have the ability to send a link to your friends and they can register themselves on your team!

I am an organization. How do I get my employees involved?

You can do prepromotion prior to the day to educate and engage your employees. Day of, you can host a registration table within your workplace and/or create teams to go inside or outside the organization to get people signed up. Register as an organization through our Registration Page (launching this summer) and a LiveOnNY representative will work with you to ensure your participation is successful.

I registered as an individual and I want to change my registration to be a member of a team. Is that possible?

Yes, simply email Abigail Mallick at amallick@liveonny.org with your name and she will help you adjust your registration.

How long does Organ Donor Enrollment Day last?

Everyone will have all of October 4 to collect enrollments.

Do enrollment forms collected outside 9am to 5pm count?

Enrollments must be garnered on Wednesday, October 4, 2017 and dated as such to count for the competition. Any enrollment forms with a date other than October 4 will not be counted toward a team's total submission, but will still be added to the New York State Registry.

What if it rains? Will the competition be rescheduled?

The competition will take place rain or shine on October 4.

What do i need to wear on organ donor enrollment day?

All participants are asked to wear the provided Organ Donor Enrollment Day t-shirts. It will be easier for people to spot you to sign up! Other than the t-shirt, dress comfortably for the weather and your activity.

Why is there an organ donor enrollment day?

There are over 10,000 New Yorkers currently waiting on the organ transplant list. Every 18 hours a New Yorker dies waiting for an organ transplant. 9 out of 10 New Yorkers support organ donation but only 28% are acutally enrolled on the New York State Registry; this is well below the national average of 50%.

 

 

I don’t know anything about organ donation, how can I learn more so I am prepared to answer questions when I register individuals?

Prior to the day, you will receive information about organ donation and New York’s need for lifesaving organs. In addition, LiveOnNY will be hosting educational webinars on organ donation. Information about these sessions will be sent through email in the coming weeks.
